The consultation form asks for contact details, an area of interest and a brief message. When you prepare an email inquiry, the information stays on this page until you choose to open your email app. It is sent to HQINS only when you send the email. A downloaded request file is saved on your own device. Do not include medical records, Social Security numbers, bank information or other sensitive details.
Using your inquiry
Information you send to HQINS may be used to respond to your request, discuss relevant insurance or benefits services, and communicate about next steps. A consultation request is not a policy application and does not bind coverage.
